2008 Long Course Meet Commitment Form

Please fill in all swimmer information then select a YES or NO response for each day of every meet. Please provide comments if you select No as a response or if you need to provide any additional information to the coaches. Once you are done making your selections, you can print the form for your records, then YOU MUST CLICK THE SUBMIT BUTTON. Swimmers are automatically entered in all the applicable meets on the meet calendar for their group and escrow accounts will be charged accordingly unless this form is filled out and submitted by Monday, April 21. LC meets fill up very quickly. Coaches must submit meet entries very early to secure our spot in the meets, so there will be NO EXCEPTIONS to the cutoff date.
